Advancing Healthcare Excellence Through Physician Career Alignment and Transformational Leadership

Abstract
Healthcare systems continue to grapple with physician burnout and its consequences for patient outcomes, organizational performance, and community well-being. When physicians become disconnected from their professional purpose, the resulting disengagement can compromise care quality and institutional stability. Guided by transformational leadership theory as articulated by Burns (1978) and Bass (1985), this qualitative single case study entailed exploring how middle and senior-level healthcare managers cultivate meaningful physician engagement to mitigate burnout. Seven experienced healthcare managers from a Central Pacific United States healthcare organization participated in semistructured interviews, supplemented by reflective journaling and analysis of organizational documents and publicly available materials. Data were analyzed using thematic analysis and constant comparison techniques. A dominant theme emerged: the strategic importance of structured career advancement and individualized professional development. Findings suggest that leaders who intentionally explore physicians’ aspirations and align them with organizational goals foster higher engagement, improved patient care, and stronger institutional outcomes. Strengthening physician engagement practices may support not only clinical excellence but also broader community health equity initiatives.
